To request a sacrament certificate from La Purisima, please complete the form below and submit to:

PLEASE NOTE:
- Parents must submit requests for minors
- If over 18 years, the individual must submit the request
- Preparation of the certificate will be approximately one week and there is a suggested donation of $5 upon pickup
